Claims History 

  • Download the Authorization to Release Claims and Insurance History form
  • Complete the top of the form authorizing the release of the information 
  • The Ministry Name is "Providence, Washington DC" 
  • Indicate if the information is to be released to you or name the person/entity to whom the information is to be released 
  • If releasing to an outside entity, attach the letter or email you received requesting the information 
  • Email the release and if applicable the letter from the entity requesting the information to: credrequest@ascension.org 
  • Allow 10 days for the request to be processed.
